Dear Jr./Sr. High School students and parents,
We are dedicated to fostering a safe, healthy, and supportive environment that enhances every student's ability to learn and thrive.
In partnership with families, we strive to maintain the wellness of our school community through proactive care, open communication, and diligent health management. We look forward to working with your children to keep them healthy throughout the school year.
To help us achieve this mission and provide the highest level of care, we ask parents and
guardians to partner with the school nurse by providing accurate, up-to-date health information.
Your collaboration is vital to the health and academic success of our students. Please do not

Effective 1/31/21, all health examinations performed for public school students when they are a new student in a school district and when they enter Pre-K or Kindergarten, and grades 1, 3, 5, 7, 9, and 11, must be documented on the NYS Required Health Examination Form or an electronic health record equivalent form which may not look the same, but presents the same information in the same order as indicated in the instructions - pursuant to Education Law:

All incoming 7th, 9th, and 11th graders are required by NYS Education Law to have a current physical exam. All students in any grade who participate in sports are also required to have a physical exam. This exam expires one year from the date that it was originally done.
